Healthy Families is an evidence-based home visiting program that provides comprehensive services to mother and child from the prenatal stage to as long as 3 to 5 years after birth, and focuses on positive child development and school readiness outcomes among others.

Pregnant women who are screened positive for high risk by the Health Department’s Administrative Care and Coordination Unit and who reside in one of the selected zip codes (21206, 21220, 21221, 21222, 21224 or 21237).
