Program Summary
A program where family navigators provide information and referrals, one-on-one support, education, coaching, and assistance with completing applications and forms and care plan guidance with a special emphasis on informal and natural supports.
Target Population
The LAM provides coordination and utilization of existing resources and supports; access to services by families and supports the CME.Targets parents, caregivers and/or family members of at-risk special needs children.
