A coordinated system of care using an on-line web-based information system supported with a telephone Warm Line and a System Navigator to assist Anne Arundel County families with children with intensive needs in gaining access to information, services and resources.
Target Population
Families with children (ages birth-21) who are seeking up-to-date information about resources and services available to support their children with intensive needs and that may require some supportive help.
