Program Summary
Family Navigator program supports families by (1) providing unbiased information that allows them to assess what will work best for them; (2) providing a structured context within which they can develop their own plan; and, (3) maintaining a constant focus on the need to build parental and familial capacity to plan and access services. In FY 16, the alignment to Community Schools will be strengthened so that these resources can be more broadly disseminated, the information on continued gaps in resources will inform our continued work, and Community Schools will learn how to establish peer to peer support networks within their schools.
Target Population
Families in Baltimore City with children with developmental disabilities and/or behavioral/mental health needs. Referrals for services are made from a multitude of sources across the City, including hospital and school staff, DJS/court staff, mental health providers. Parents can also self-refer for services. Family Navigators are actively engaged in marketing the availability of the program.
