LAM - Navigation Annual
Program Summary
A Family Navigator
will assist families in accessing services based on their Family Plan. The Family Navigator will make linkages,
provide support and advocacy, and follow up with the family. Information and referral will be
provided by screening calls, providing
information and referral, family-to-family support, and educational services
Target Population
All families in Somerset County with children under 21, with navigation services for those families whose children have intensive needs.
