System/Family Navigation (Worcester County FY16 Annual)
Program Summary
For those families who need additional assistance beyond a simple referral, the System Navigation function is utilized within the LAM. In System Navigation, the family is assisted with identifying strengths and needs and obtaining necessary services. The individuals who provide the system navigation are available to families as they present themselves. Some families may simply need assistance in systems navigation-identifying specific resources and gaining access to services.
