Program Summary

A two generation approach to address identified risks in vulnerable, pregnant women, infants and children up to age three. Services include home visiting, ongoing care coordination and support, psychosocial, nutritional and smoking cessation counseling, childbirth, and breastfeeding.

Target Population

Pregnant woman and mothers with infants or children up to 3 years old; specific focus on North County, families impacted by incarceration, and on African-American families due to racial disparities in birth outcomes.
