First half of the FY2018

The Community Service Center (CSC) continues to receive a vast number of referrals and is actively utilized by various community agencies and groups in Worcester County. The staff working with the CSC has continued to improve tracking information through a general inquiry data form which allows for more accurate numbers and referrals to be made. Overall, clients utilizing the CSC are satisfied with the services and referrals that are made to partnering agencies. 

Worcester Youth and Family is confident that they will meet targeted numbers for FY18. 

Second half of FY2018

During the second half of fiscal year, clients and community members continue to access the Community Service Center. 

Reflecting on FY2018

During FY2018, the Community Service Center was accessed 3,599 times, this includes the total number of individuals that accessed the Ray Community Room and the Conference room. 12,099 referrals, inquiry forms, clients served and clinical hours were provided during FY2018. The Community Service Center continues to be greatly utilized by the community and agency members. Staff has improved tracking information captured on the general inquiry form to prevent duplication and more accurately obtain data.

The Community Service Center (CSC) is a walk-in, single point of access facility serving as a consumer portal for integrated and coordinated healthcare. The CSC communicates with other community supports to provide resources to individuals or families in need. Resources obtained through the CSC include on-site mental health services, navigation services, middle school aged youth groups, support groups, life skills workshops, parenting education, and English as a Second Language (ESOL).

The target population for the Community Service Center (CSC) is Disconnected Youth or youth at risk of becoming disconnected; as well as children, youth, individuals and families who are living in poverty, those that are homeless, or at risk for homelessness.
