Story Behind the Curve

 Year-end: By year end, TCYSB had provided 523 counseling sessions, for a total of 273 cases: 80 individuals, 80 families, and 112 in formal groups. There were 79 informal cases. In the final quarter 26 youth showed improvement in CAFAS total score of 20 points, for a total of 79 (81%) at year end. The average score at initial intake was 42, compared to 15 at exit, with a difference of 27, which is significant, reflective of an increase in functioning level. The continued use of trauma-focused cognitive behavioral therapy appears to have a significant impact on participants' functioning. This emphasis on screening for trauma has proven to be effective in addressing barriers that disconnected youth face. Further, for children who are impacted by parental incarceration, the ACES screening is a guide for effective treatment planning.

Partners

Both Calvert County Public Schools and the Department of Juvenile Services provide referrals and space for individual and group counseling, reducing the barriers that transportation in a rural area presents.  TCYSB participates in the Multi-Disciplinary Team meetings at the Department of Social Services to address needs of at risk youth and families. TCYSB has partnered with the Maryland Family Network to provide the Strengthening Families Program to parents of children 0-5 years old, and TCYSB is partnering with the Judy Center to host the Cafe's.  TCYSB also worked with the Calvert County Health Department Pregnancy and Tobacco Cessation Help (PATCH) Program to be a marketing portal for the state quitline and the local cessation program. This expanded TCYSB's reach significantly: e.g., one Facebook ad that described the impact of nicotine on children reached 22,760 residents, and resulted in 439 reactions. The anti-nicotine campaign resulted in  222 youth over 18 years old reaching out for information and were referred to the Health Department. The addition of nicotine screening at intake allowed for education and early intervention not only with pregnant women, but with all youth.

What Works

The use of Cognitive-Behavioral Therapy, and facilitation of the Managing Emotions Group have a positive impact on functioning as demonstrated through the CAFAS scores.  Engaging families in this treatment modaility allows clients to work on repairing issues within the rest of the family. The addition of the ACES (Adverse Childhood Experiences) screening provided greater insight into participants' trauma level and helped inform trauma-focused treatment planning, particularly in cases where children were affected by family incarceration. TCYSB continued to administer the Ready4Life-SOMD web-based portal (website, social media) that links disconnected youth and service providers, lising employment opportunities, job fairs, and resources that are available to youth. 167 Calvert County youth contacted  the Resource Coordinator via the internet and were linked to community resources.

Data Discussion

The Youth Intervention Program uses the Child and Adolescent Functional Assessment Scale (CAFAS) to assess the degree of impairment in youth with emotional, behavioral, psychiatric, or substance use problems. The CAFAS provides an objective, comprehensive assessment of a youth's needs that is sensitive to change over time. Using information collected during a routine clinical interview, the practitioner selects items that describe the youth’s problematic behaviors, as well as strengths and goals.  This is done for eight life domains: At School, At Home, in the Community (delinquency), Behavior Toward Others, Moods/emotions, Self Harm, Substance Use, and Thinking (assessing irrationality). A Total Score and subscale scores are generated, with higher scores indicating greater impairment in day-to-day functioning.  Therefore, as treatment progresses, lower CAFAS total and subscale scores indicate improvement. The CAFAS is uses at intake and again at discharge from the program.
