The City of Salisbury is not a contract vendor for FY19. 

 

The awarded vendor for FY18 was the City of Salisbury.

Strategy

The vendor/agency gets a healthy mix of both self-referrrals (usually word of mouth from previous clients) and referrals from third-party agencies. While predictive outcomes are rare, typically persons that reach out to the vendor when they have no other options, and they have the most barriers which take an inordinate amount of time to assist. Understandably many people come to us when their shelter stay has come to an end, and they are simply focusing on their most immediate need: "where am I staying tonight?" Once that need is fulfilled, often we will not hear from those persons until the next crisis arises, even if their solution is not permanent. The vendor is therefore, trying to direct more of their time toward those clients with more complex needs and higher barriers. 

Action Plan

Although the vendor is trying to keep the program as a open door policy with low barrier to access to services, they are continually working to create a system to better prioritize clients that have more complex needs. Due to the nature of many of their clients needs being emergent, the vendor often responds to client's need that is immediately in front of them, even if their need  is not the most pressing. The vendor continuing to examine different ways to better priortize those clients with more complex needs. The vendor is completing "warm hand offs" for those who need resources the vendor cannot assist with and are scheduling appointments by phone for those with less pressing needs. The vendor will continue to prioritize the highest need clients, and reevaluate the means by which needs are assessed. 

Program Summary

Systems Navigation services utilizing the No Wrong Door approach following a Systems of Care philosophy using the Child and Adolescent Needs & Strengths (CANS) assessment tool.  (Followed by both vendors). Services provided also include Information & Referral and Case Coordination are provided by both vendors.

The City of Salisbury will provide Navigation services to homeless persons living in the City of Salisbury through the Homeless Street Outreach & Resource Navigation Program. The City Navigator will work to deem eligibility for the City’s Permanent Supportive Housing Program.

Measurement Tool Used

The two main metrics being measured that required specific definitions were "reported success in receiving needed services or supports" and "reported an increase in their ability to adovcate for the needs of their child/children after receiving Navigation Services". The former was defined as "clients whom either the vendor completed a formal referral for or who were connected with the appropriate agency"; ie a warm hand-off. The venter currently utilizes surveys and CANS assessments for this measurement.
