Program Summary
The Family Navigation program serves as an information and referral system for families by coordinating and linking them to services and resources, providing support, encouraging families to develop their advocacy skills, and providing educational/support opportunities which foster increased knowledge, competency and relationships by and between program participants.
Target Population
Children and Youth who have either a learning and/or developmental disability and/or emotional/behavioral/medical issues that are residents of Allegany County that without intervention are at high-risk for becoming disconnected.
Participants will be referred through community agencies or may self-referral.Program Summary
