What Works

Consistent communication with CARE individuals and families makes the families feel supported throughout their process of locating resources. These families report that they have someone they can trust to connect them to the best possible resources to meet their needs. Also, continuing to build relationships with local providers helps CARE to stay abreast on opportunities and resources in the community. Lastly, educating families on requirements for housing (i.e.: minimum income necessary for housing, how to apply for housing, looking for available housing, etc.) helps families make a plan for obtaining the housing they are in need of.

Systems Navigation services utilizing the No Wrong Door approach following a Systems of Care philosophy using the Child and Adolescent Needs & Strengths (CANS) assessment tool. Services provided also include Information & Referral and Case Coordination are provided by the vendor.

 

Action Plan

In the future, it may be helpful to have families take the satisfaction survey before services are completed, therefore increasing the number of surveys completed. This will more accurately reflect the number for individuals served by the CARE program. CARE will also coordinate with local agencies to present the CARE program on a quarterly basis (ie Life Crisis, 211 Call Center, local Emergency Room, and Urgent Care Centers).

Program Summary

The CARE program provides a pathway for families in the navigation of the service delivery system. Under a “no wrong door” policy, families are able to enter the LAM through referrals from existing agencies including the care mangement entity (CME) or through individual contact in order to access all available services to support family to remain in community.

  • Homeless persons in Wicomico County and City of Salisbury

  • Families with children living in poverty

  • Families impacted by incarceration in Safe Streets areas

  • Any Wicomico County resident seeking Navigation Services

How the Program Serves the SB882 Population

The Local Access Mechanism will target the Safe Streets Area, and provide targeted mobile resources to ensure familiarity with services offered.

Measurement Tool Used

The data (# of individuals served) is gathered from Shared Village. CARE also uses a satisfaction survey that reflects the results listed so far for FY19.
