Care Coordination Bureau

The Care Coordination Bureau is committed to ensuring that Harford County residents have access to comprehensive preventative health services to improve and maintain their health and wellness.

Programs: 

• Administrative Care Coordination - The Administrative Care Coordination Unit (ACCU) is a local resource for information for Medical Assistance (Medicaid) recipients. The program increases the client's overall awareness of the Medicaid program and provides HealthChoice and Medicaid education. The program assists the recipients with choosing a managed care organization and health care provider while teaching the importance of preventive health care.
• Care Coordination Plus - Care Coordination Plus assists clients in accessing community resources and the medical care they need. All Harford County residents are eligible regardless of age or the type of medical insurance they have. The Care Coordinators work closely with the University of Maryland Upper Chesapeake Health Care System hospitals and other local agencies to plan and coordinate medical care and the day-to-day needs of Harford County residents.
• Nurse Monitoring - Nurse Monitoring provides assistance with activities of daily living, medication management, shopping, and light housekeeping to medical assistance recipients. Also, clients may be eligible for an environmental assessment, adaptive equipment, and more.
• Adult Evaluation and Review Service - The Adult Evaluation Review Services conducts comprehensive evaluations to identify services available to help individuals remain in the community, or in the least restrictive environment while functioning at the highest possible level of independence and personal well-being.
• SENS - Substance-Exposed Newborns
• Childhood Lead and Asthma Program - The Lead and Asthma Program (LAP) is a home visiting program that works with children ages 0-19 in Harford County to reduce symptoms of moderate to severe Asthma and to help lower elevated Blood Lead Levels ≥3.5µ/dL. LAP provides educational materials, HEPA filter vacuum cleaners, doormats, pest management kits, and green cleaning supplies to families in order to reduce these problems.
