Why Is This Important?

Diabetes is a serious chronic disease which is costly for individuals as well as for communities, businesses, governments, and others. Currently, an estimated 10.8%* of Connecticut adults have diagnosed diabetes (types 1 and 2), or approximately 312,000 adults, with higher rates among racial and ethnic minorities and adults with household incomes <$25,000/year (2021 Behavioral Risk Factor Surveillance System data). Diabetes can lead to various complications such as heart disease, stroke, kidney disease, nerve damage, blindness, dental issues, premature death and lower extremity amputations. However, individuals, communities, health departments, the health care system, and others can partner to prevent or delay type 2 diabetes and its complications by ensuring that diabetes self-management and prevention services are accessible to all.

The Department of Public Health’s Diabetes Prevention and Control Program partners with local health departments, hospitals, community-based organizations, and community health centers and other health care organizations to increase the number people receiving diabetes self-management education and attending diabetes prevention lifestyle change programs. Diabetes self-management education services help persons with diabetes work with their providers to become empowered self-managers to achieve control of blood sugar, blood pressure, and other risk factors which can help mitigate or delay many of these complications. Diabetes prevention lifestyle change programs help persons who have prediabetes or who are at risk for type 2 diabetes change certain aspects of their lifestyle, like eating healthier, reducing stress, and getting more physical activity, with the goal of preventing or delaying type 2 diabetes.
